Focused shockwave therapy (fESWT) is a highly precise, non-invasive orthopedic treatment in which bundled, high-energy sound waves are directed with pinpoint accuracy into deeper tissue layers. In contrast to radial shockwave therapy, focused shockwave therapy works at a clearly defined depth, allowing even deeper structures such as tendon insertions or calcium deposits to be treated effectively.
Areas of Application of Focused Shockwave
Therapy
A focused shockwave device enables:
- Precise control of penetration depth (e.g. 0.5–6 cm)
- High-energy impulses that stimulate regeneration and pain reduction
- Tissue-sparing, pinpoint application without energy loss
- Treatment of hard-to-reach areas that are otherwise difficult to access
Typical orthopedic indications for treatment with a focused shockwave device:
- Calcific shoulder tendinopathy (tendinosis calcarea)
- Targeted breakdown of calcium deposits in the rotator cuff
- Particularly suitable because the shockwave can be focused precisely on the calcified area
- Heel spur and plantar fasciitis
- Deep treatment of the plantar fascia in the foot
- Improved circulation and pain reduction at the insertion site
- Achilles tendon complaints
- Focused effect directly on the painful tendon area or deep insertion
- Effective even in chronic irritation
- Patellar tendinopathy (“jumper’s knee”)
- Precise treatment of the tendon insertion below the kneecap
- Especially helpful for physically active patients
- Greater trochanteric pain syndrome
- Treatment of the deep gluteal tendon insertions at the hip
- Successful in long-standing, treatment-resistant complaints
- Myofascial pain syndromes in deeper muscle layers
- Release of deep trigger points that are difficult to reach with manual therapy
Your Benefits of Focused
Shockwave Therapy
-
Non-Invasive Treatment
Focused shockwave therapy is performed without surgery, injections, or anesthesia and is therefore particularly gentle.
-
Targeted Deep-Tissue Effect
The shockwaves are focused precisely on the source of pain and also reach deeper structures.
-
Activation of the Body’s Natural Healing Processes
The treatment can promote circulation, stimulate metabolism, and support regeneration of the affected tissue.
-
Short Treatment Duration
A session usually takes only a few minutes and can be easily integrated into everyday life.
-
No Downtime
As a rule, patients can resume normal activity immediately after treatment, and longer recovery periods are usually not necessary.
-
Individually Tailored by a Physician
Shockwave therapy is used only after prior orthopedic assessment and an individual medical indication has been established.
Frequently Asked Questions About Focused Shockwave Therapy
Is focused shockwave therapy painful?
The treatment may be perceived as uncomfortable depending on sensitivity and the treatment area, but it is generally well tolerated. I adjust the intensity individually. Local anesthesia or general anesthesia is usually not necessary.
How many sessions are necessary?
The number of sessions depends on the condition being treated, the duration of the symptoms, and the individual response to therapy. As a rule, at least 3 sessions at intervals of 1–2 weeks are recommended. The duration of each treatment is at least 15 minutes. In many cases, several sessions at spaced intervals are required. I will discuss this with you individually.
When can improvement be expected?
Some patients feel relief after the first treatments. In many cases, however, the effect develops gradually as the body’s natural healing processes are stimulated. An immediate effect cannot be expected in every case.
Are there any side effects or risks?
Focused shockwave therapy is considered a safe procedure. Occasionally, temporary redness, tenderness to pressure, or mild pain in the treated area may occur. Serious side effects are rare.
Will health insurance cover the costs?
Focused shockwave therapy is generally not covered by statutory health insurance. In my private practice, billing is based on the German scale of fees for physicians (GOÄ). I will inform you transparently about the expected costs before treatment begins.
Can I resume normal activities after the treatment?
As a rule, you can continue your normal daily activities after the treatment. Depending on the condition, it may be advisable to temporarily adjust sports or other intense physical activities. I will discuss the appropriate recommendations with you individually.
